Facility Use Request

Please complete this form if you or your organization is interested in utilizing the Department of Physical Therapy’s lab classrooms for meetings or continuing education courses. The Department’s Administrative Assistant will be in contact with you regarding your request. Scheduling depends on course relevance to the profession of physical therapy, room availability, and approval by the Department of Physical Therapy Chair.
